Baden-BadenLaparoscopic Repair of umbilical, incisional, epigastric and parastomal hernias
"700,000 patients undergo a laparotomy per year in Germany resulting in incisonal hernias at an estimated rate of 15%. So 105,000 patients develop an incisional hernia per year. The conventional repair of incisional hernias is complicated by recurrence rates sometimes exceeding 20% despite mesh-based techniques and a high frequency of wound complications. The laparoscopic approach seems a very promising step to improve the outcome of incisional hernia repair as demonstrated by some short-term and medium-term results in the literature. Nevertheless laparoscopic repair is barely used in Germany mainly due to high costs. The increasing number of appropriate meshes caused a striking prize reduction since 2004 which should support the broader introduction of the technique. DynaMesh-IPOM available since October 2004, combines biological compatibility, elasticity comparable to the human abdominal wall, handling-properties, and acceptable costs. After several hundered patients who underwent laparoscopic repair in our institution since September 1999 and our own experience with different meshes we are convinced of the superiority of laparoscopic repair especially in combination with the newly developed mesh. In order to introduce interested surgeons in the laparoscopic repair of incisional hernias we offer again, together with FEG Textiltechnik mbH, Aachen, workshops with 3-4 live procedures. The aim is to demonstrate our technique evolved during the years as well as outlining some tricks which alleviate the surgical live and enabling the participants an advanced start of laparoscopic repair of incisional hernias."
